This fee includes admission to the congress, the Welcome reception on Thursday June 18,a copy of the programme book and coffee/tea/lunches during the congress. Please note that the congress dinner is not included. Method of payment 
Full payment in Euro should be made when you register. Registrations by fax will be accepted with credit card payment only. Cancellation policyCancellations received by February 1, 2009 will be refunded less 35 Euro administrative charges. Cancellations received after February 1, 2009 are non-refundable. Cancellations must be in writing and sent to the Congress Secretariat eitherby mail, fax, or e-mail. If for any reason the congress is cancelled, the 5th International Congress on Shwachman-Diamond Syndrome, is not responsible for covering airfare, hotel, or other costs incurred by congress registration. Confirmation of RegistrationAfter registration you will receive a confirmation (by email) with all the relevant information. You will receive your programme book and badge on site. Name ChangeShould you be prevented from attending the congress, you will be given the opportunity to send a colleague in your place. Name changes are possible until March 1, 2009. Should the changes occur after March 1, 2009, there will be a charge of € 40 per name change. LiabilityThe participant acknowledges that he/she has no right to lodge claims against the organizers should the holding of the meeting be hindered or prevented by unexpected political or economic events or generally by force majeure, or should the non-appearance of speakers or other reasons necessitate program changes.
